Well, The "How" is quite simple, an authority figure pressed that magical button called Ban

Actually, it's called something different. "Ban IP/User" for Admins, "Ban User" for Super Mods. :P

So, your first ban. What happened there? And when?

Well, this Team was very monitored , and I dont remeber who it was with, but nonetheless we had setttled it and I didnt want anyone to get into to trouble soo I attempted to cover our tracks
